How to Become a Client at
Mountain Lane Home Care

Intake and Application

The first step of the process is connecting with our enrollment team to learn about our program and discuss your needs.

Case Manager Involvement

Our Enrollment Specialist and Client Administrator will work with you and your Medicaid case manager to develop and manage a personalized service plan.

Start of Care

Your case manager will provide a start date which is when your caregiver can begin services!

Ongoing Support

You will have dedicated staff members who facilitate a close working relationship between you and your case manager.

Once you have discussed the services and frequency of services needed, we will work with you and your case manager to choose a start date for services!

How long does it take to get started?

The time it takes to get an approved service plan and start date can vary, typically care can start within a few weeks.

Can a family member get paid to be a caregiver?

Yes – absolutely! See information above on how to get started.

How do you help families after enrollment?

Our Client Administrator is available for service plan meetings and acts as a liaison for you and your case manager for monthly, quarterly, and/or annual service plan meetings.
